Wia Training Provider Application Instructions

Section 1: Training Provider Information

All Section 1 items are required for application processing. This information will be used to compile the Arizona WIA Statewide Consumer Report System and Eligible Training Provider List.

Item 1 Training Provider Name This is the name that will appear on the Web site.
Items 2 – 7   Self-explanatory
Item 8 Administrative ContactThe individual listed will be the primary contact for questions regarding the application and all supportive documentation
Item 9 Description of the Training Provider This is a brief description of the training provider, not to exceed 50 words.
Items 10 – 13 Self-explanatory

Section 2: Training Program Information

Item 1 Program NameThis is the name that will appear on the Web site.
Item 2  Self-explanatory
Item 3 Description of the Training Provider This is a brief description of the training program, what it covers, what graduates come away with, etc.
Item 4 ParkingIf parking is available, the applicant should specify what the cost is per period of time, i.e., month, quarter, semester, year.
Items 5 – 7  Self-explanatory
Item 8 Type of Training ProgramThis specifies the type of training program offered.
Items 9 – 13 Self-explanatory
Item 14Length of Time to Complete Training ProgramTraining provider is required to specify amount of time in weeks, months, or years.
Item 15List of OccupationsThis is a list of potential occupations that a student might pursue after completing the training. The applicant is required to provide extra documentation from employers if there are occupations listed here that are not on the Demand Occupation List.

Section 3: Training Program Costs

Item 1 Detailed Cost Information This table details cost information per participant. The applicant is required to provide current information on tuition and required fees and related program costs.

Section 4: Performance Statistics

Item 1First Time CertificationIf this is the first time the applicant has applied for certification as a training provider under WIA for this specific training program, they must complete Item 2.

If it is not the first time they have applied for certification under WIA, they are required to skip Section A: Initial Certification and complete Section B: Re-certification ONLY.

Item 2HEA and NAA Training ProgramsNOTE: Only applicants who indicated in Item 1 that this is the first time they have applied for certification as a training provider of this specific training program under WIA should complete this question.

If the applicant indicates that their training program either leads to a certificate or degree under Title IV of the Higher Education Act OR is registered under the National Apprenticeship Act, they are NOT required to complete Section 4: Performance Statistics. They should skip Section 4: Performance Statistics and proceed directly to Section 5: Assurances.

If the applicant indicates that their training program DOES NOT lead to a certificate or degree under Title IV of the Higher Education Act or IS NOT registered under the National Apprenticeship Act, they must complete Section 4A: Initial Certification ONLY. They are not required to complete Section 4B: Recertification.

A: Initial Certification

Participant UniverseActual number of individuals who enrolled in the training program.
b.
Program Completion Actual number of individuals who completed the training program.
c.
EmploymentActual number of individuals who completed the training program and obtained unsubsidized employment (work at which the pay is not supplemented by other sources).
d.
Related Field EmploymentActual number of individuals who completed the training program and obtained unsubsidized employment (work at which the pay is not supplemented by other sources) in an occupation related to their training program.
e.
Average Hourly Placement Rate Average hourly wage at placement in employment for all individuals who completed the training program

Item 3Incomplete InformationThe training provider must provide a justification for any data that is incomplete or missing. They must also describe how they will collect the missing data so that it is available for recertification purposes

B: Recertification

This information should be filled out only by those training providers applying for annual recertification.

Item 1 Performance Data:Training providers are required to provide the following performance data based on ALL individuals who enrolled in the training program.
a.
Participant UniverseActual number of individuals who enrolled in the training program.
b.
Program CompletionActual number of individuals who completed the training program.
c.
EmploymentActual number of individuals who completed the training program and obtained unsubsidized employment (work at which the pay is not supplemented by other sources).
d.
Related Field EmploymentActual number of individuals who completed the training program and obtained unsubsidized employment (work at which the pay is not supplemented by other sources) in an occupation related to their training program.
e.
Average Hourly Placement Rate Average hourly wage at placement in employment for all individuals who completed the training program.
Item 2 Performance Data: Applicants are required to provide the following performance data based on ONLY WIA individuals who enrolled in the training program.
a.
Participant Universe Actual number of WIA individuals who enrolled in the training program.
b.
WIA Completion Actual number of WIA individuals who completed the training program.
c.
WIA Employment Actual number of WIA individuals who completed the training program and obtained unsubsidized employment (work at which the pay is not supplemented by other sources).
d.
WIA Retention Actual number of WIA individuals who completed the training program, obtained unsubsidized employment (work at which the pay is not supplemented by other sources), and are still in unsubsidized employment six months after the first day of employment.
e.
WIA Hourly Wage Average hourly wage six months after the first day of employment for WIA individuals who completed the training program.
f.
WIA Certificate Actual number of WIA individuals who completed the training program and attained licensure, certification, an academic degree or its equivalent, or other measurable skills.

Section 5: Assurances

Item 1 Link to Arizona WIA Statewide Consumer Report System and Eligible Training Provider List Web Site Training provider should specify their Web site address if they would like their home page linked to the Arizona WIA Statewide Consumer Report System and Eligible Training Provider List Web site.
Item 2 Assurances A check mark next to each listed item indicates the training provider can provide the item upon request.
Item 3 Certification Regarding Lobbying Specifies rules regarding lobbying.
Item 4 Authorized Signature Signature of the individual authorized to certify application as accurate. Must include typed/printed name of signatory, signatory’s title, name of organization and date signed.
